A/N I'm sorry in advance ⚠️ANGST, FEELS⚠️
Dean and Cas have been married for 47 years. They've both grown old and grey with the years, as Cas gave up his grace to grow old with Dean.
Now, though, Dean wishes he didn't. Cas is lying in a hospital bed in end of life care. He's been struggling with leukemia for the past few years, but it just caught up to him recently.
He's going to die, and Dean knows it.
Dean sits at the side of his angel's bed, holding his hand all through the night and morning of the next day. He's heartbroken that Cas is leaving him soon, but he wants to be strong for him.
Suddenly, Cas gasps and Dean turns his head to the ex-angel's. "Baby, what is it? Do you need something?" Dean asks, frantically, pressing a nurse button. Nobody comes, though.
"Dean, I have to go." Cas says simply. "It's time." He explains. Dean let's his tears flow freely and sadly nods at Cas. He knew this would happen.
"I will always love you, Castiel. No matter how long it takes me to get up there with you, I will love you every single day of my life, and even more after." Dean says, crying softly.
Cas reaches up to Dean's face and pulls him down into one last kiss. Dean sobs through the kiss, not wanting to let his lips leave his husband's. Cas' lips have never been totally soft, but now, it's like they're silk.
Dean smiles slightly until he feels Cas' lips stop moving. Dean sighs, sadly. He knows this is it.
"Goodbye baby, I'll see you soon." Dean says, hugging Cas' body that has now gone limp.
"I love you, Dean." Cas whispers, his heart monitor flatlining. Dean sobs for hours, not knowing what to do. Even after they took Cas' body off to the morgue, he sits in the empty room, wishing for more time. Any time would be greatly appreciated by Dean.
Dean pulls out his phone, taking a deep breath, and calls Sam. "Sammy," he says sadly.
"Dean." Sam replies, knowing what Dean will say.
"He's gone, Sammy." Dean cries, not wanting to admit it.
Sam replies with something, but Dean can't hear it. He can't hear anything other than Cas' final breath and the heart monitor. He never knew how comforting the heart monitor beeping rapidly was until now.
Dean walks out of the hospital, holding Cas' iconic trench coat and his glasses. He was never really fond of those glasses, but as he aged, he needed them do to the simplest of tasks.
Dean drives to the bunker, his and Cas' kids awaiting his arrival. They're all nephilims, and they should outlive everybody here on earth, but now they're wishing they wouldn't.
Their oldest son, Dalton, hugs Dean tightly as he walks through the doors. Dean cries into his son's arms and his daughter, Claire, (named after Claire Novak), joins them.
The three hug for what seems like forever, but Dean pulls back eventually. "Your dad and I will meet again soon, and I know it will be hard for you guys, but you have to stay strong for me and him, okay? Promise me that you two will live your full lives and be happy even without us." Dean says, looking into his kids' eyes.
"We promise dad." They both say in unison.
"Thank you, kiddos. You know your dad loved you very much. He went peacefully, and he knew before it was happening. He said his goodbyes and was content as he left us." Dean explains, the three walking over to Sam.
"I'm sorry, Dean." He says, pulling his brother into a hug. Dean sighs and hugs Sam back, sniffling into his shoulder.
"He's happy. I can feel it." Dean says, smiling softly. Cas' part of his soul may have went up to heaven, but he can feel the happiness in his husband's mind. He can tell that he's okay with where he is, and he's waiting patiently for Dean to join him in paradise.
Dean sighs, pulling back from the hug. He walks to his bedroom and shuts the door, laying down in his bed. It suddenly feels so much bigger without his husband in it. Dean hugs Cas' pillow and takes in the natural scent of Cas. At first it was weird having another man's scent in your bed, but now, once it's gone, Dean knows he'll be lost without it.
Dean falls asleep, saying, "goodnight Cas, I love you." To nobody. He knows no one is there, but without saying it, he can't sleep.
Dean dreams of the old days with Cas. Way back in 2009 when they met. They had very different views on the world, but somehow, by 2017, they knew that they loved each other. They got married way after, after years of dating, and had their first kid in 2023. Then Claire was born in 2027. After that, Cas could see that Dean was aging, so he decided to give up his grace to age with his husband.
About twenty years later, in 2041, Cas was diagnosed with stage 2 leukemia. They tried treatment for years, but it didn't work. They stopped treatment about 3 years after and decided to live out the rest of their days in happiness, not worrying about the cancer, monsters, or anything else. They went on picnic dates in a nearby park, spent time with their kids at the bunker, and accepted Cas' fate.
Every night since Dean found out about Cas' cancer, he prayed that when Cas goes, he'd go too. And that's exactly what happened.
When Dean woke up, he was surrounded by gorgeous flowers and vines, strung up around a huge white bed. He rolled over and smiled as he saw his husband, young again, laying next to him, staring at his features. "Tell me this is real." Dean says, pulling a hand up to Cas' smooth, unwrinkled face.
"It's real." Cas says, smiling. "This is our own piece of heaven, Dean. I wasn't even here two hours before you got here, what happened?" Cas asks, cuddling Dean close to him.
"I prayed." Dean says and Cas looks shocked. "Every night since you got diagnosed, I prayed that when you go, I'd go too." Dean admits, tears falling down his cheeks.
"What about Dalton and Claire? Sam?" Cas asks, worrying about their loved ones down on earth.
"I told them that I would be gone too, soon. They promised me that they'd live their lives happily without us. They're going to be okay, Cas. Don't worry. Just kiss me." Dean says, smiling at the thought of their kids and brother living happily down on earth while they live happily up in heaven.
Cas smiles too and leans in, grabbing Dean's cheeks. Soon, their lips have been together for so long that they're getting sore. They haven't kissed like this in years. Their old age took too much out of them, they never got to do what they desired anymore.
So that's exactly what they did.
For the rest of eternity, they lived happily in heaven together, doing whatever it is that they desire.
