As you can see the Galaxy warps are in direct correlation with the stars. Lapis didn't go to Homeworld. She was going to the star above it. She was going somewhere other then Homeworld but she was captured along the way.
In "Winter Forecast" the gems were using the shooting star to blow up one of the warps. They accomplished this because in "Friend Ship" one of the warps were gone. The animators made a mistake and got rid of the wrong one though. During the battle with Peridot the one they got rid of eventually came back but the one that Lapis tried to travel to was gone. They didn't want Lapis to come back with whatever lives on that planet.
