A/N : TW for some dysphoria and a brief mention of Todd's anorexia
"Todd, are you busy right now?" Rafael asked a bit quietly as he walked into the kitchen, wearing only a pair of pajama pants and his binder. Todd was glad that he felt comfortable enough around him to wear such little amounts of clothing when he usually swamped himself in that hoodie to help with the dysphoria. He understood the urge of course. While he wasn't trans, he still had body image issues and would rarely take his shirt off for fear of people judging his body. He was just glad that Rafael felt safe enough around him to dress less strictly.
"No, I am not" Todd replied with a small smile. "We have work off today, remember?" he asked a bit rhetorically because he knew that Rafael was fully aware of this. It had been pretty much all he'd talked about the previous day because Shawn hardly ever gave his servants an actual free day. Sure, some days were less eventful than others, and it's not like he and Ida had missions every single day or anything, but there was still always something. But not today, no. Today was free.
"I know, I just..." Rafael trailed off with a shrug. "I wanted to make sure I wasn't bothering you or anything cause I kinda have a favor to ask, but if you're busy then-"
"You're not bothering me" Todd cut him off with a smile as he walked over to the nervous boy. "What did you need?"
"...Can you do my T shot?"
"Of course" Todd replied immediately. It's not like this would be his first time doing it or anything. He actually did it fairly often, especially since Dr. Asclepius had changed the blue-eyed demon's dose from once a month to once every two weeks. Ida would also do his shot a lot (he helped with her estrogen shots pretty often as well so it was like a mutual understanding between them), but she wasn't always available and Rafael hadn't done it himself since the early months of taking the hormone due to him accidentally hurting himself too many times. "Do you have the needles or...?"
"Yeah, they're in my room" Rafael replied, seeming a bit relieved at Todd agreeing. He had no reason to be nervous because it's not like Todd would've ever said no or anything, even if he was busy, but he still worried about it. "I would rather you do that in there since it's more private if that's okay"
"Sure thing"

Todd sat on his coworker's bed and curiously watched as Rafael set everything up.
He first pulled out the vial of testosterone that Dr. Asclepius had prescribed him. It honestly amazed him how that woman seemed to be at least sufficient in practically every field of medicine, though she kind of had to considering she was the only qualified doctor. Pretty much every other doctor quit work once they got to Hell, deciding they didn't want to be tied down to the profession for eternity. Asclepius was just the lucky one who Lucifer swooped in and forced to work.
He took out an alcohol pad and wiped off the top of the vial. It wasn't a necessary step (it's not like he could actually get hurt from not doing it considering he was already dead), but it would make him feel wrong if he didn't do it, so he still took that tiny precaution each and every time. He then took the syringe out of its little packaging along with the thicker of the two needles he had on hand- The drawing needle.
He screwed the needle onto the syringe and poked it into the rubber top of the vial. He pulled it out a whole lot then pushed most of it back into the vial until it was at his correct dosage. He pulled the needle out of the glass and flicked his pointed finger at the syringe a few times to get the air bubbles out, wincing as the sharp motion sent little jolts of pain up his finger before capping the needle and removing it from the syringe.
He then picked up the smaller needle (which he so affectionately referred to as "the stabbing needle") and screwed it onto the syringe, handing it to the red-eyed demon who accepted it with a kind smile. Rafael then grabbed the other alcohol pad and bandage and sat next to his boyfriend, avoiding his gaze. That rang an alarm in Todd's head as he nudged the other boy gently. "Are you nervous?" he asked kindly. "You know I won't hurt you, right? I know what I'm doing"
"It's not that" Rafael replied honestly. It was true. He'd had a really bad fear of needles in the past (and he still did when it was in a medical setting), but he'd had this specific injection so many times that he never worried about it unless it was someone who hadn't ever done it before. Even then, his nerves weren't that bad. Not so bad as it had once been, at least. Slight shakiness at worst. "I just... I feel like this is just an unpleasant reminder that you're dating a... You know"
"Rafael, you're a boy" Todd replied firmly. "I see you as no less of a guy than Jasper or Randall or any of the other men I've ever been attracted to" he continued before Rafael could retaliate. "You seem so proud of your transness outside of our relationship, and it's honestly kind of sad. I hope you know that you being trans doesn't bother me in the slightest. If it did, we wouldn't be in a relationship, would we?"
"I just... Feel like you shouldn't be dealing with this kind of thing"
"If I had a cis boyfriend who needed testosterone injections because of low T-levels, and he needed my help with the injections sometimes, would you say that it's unfair to me or that he isn't a real boy because of it?"
"No, but-"
Rafael's protest was abruptly cut off by a sharp feeling in the underside of his arm as Todd injected him. The blue-eyed demon couldn't help but smile, feeling some of his insecurities of the situation fade away as Todd slowly pushed the liquid in with so much care.
He finished the injection and gently put the bandaid over the small hole from where he had injected the other boy, then placed a kiss on his cheek as he capped the needle again and handed it to him. "I love you, jaanu" he said affectionately. "Don't worry so much"
"I love you too, Todd"
