He is intersex
Historically, Prussia had always been referred as 'she' while he himself is referred to as 'he'. It messed up with his feelings and he never exactly knew what sex he now is.
Only with the discovery and recognition of intersex he had something to identify. Still, he usually describes himself as a boy and also doesn't really like being called 'she'.
