This refers to the Butterfly Project:
When feeling the urge to self harm you instead draw a butterfly onto your skin. You then name it and have to let it fade naturally, if you scrub it off or self harm while it's still visible it "dies".
This won't help everyone, but for some it does. Also, butterflies have become a symbolism for self harm (recovery) and they're a common tattoo for people who once struggled with this.
