The street's Angel Doctor:
Person A is a kind hearted doctor that runs a small nonprofit community clinic for the poor and marginalized. The clinic only has 7 employees and all those involved offer their help and time for free. As such, there are many with complicated or unknown backgrounds but Person A doesn't pry into it. They instead help and teach some of their volunteers (as others already have the training) about clinical medicine such as first aid, phlebotomy, wound care/treatment, and suturing, minor surgery, etc. In turn, they apply those skills for patients in need. Moreover, Person A often goes on walks around notorious/dangerous areas where they search/find patients in need. As such, nearly all gangs/groups allow Person A to go about as long as they don't get in the group's way. Considering many members come from poor/difficult backgrounds, they appreciate A's honest care so they let them be.
One particularly cold and rainy night in late autumn, Person A found a trail of fresh blood leading to a dead end alleyway. Concern and self preservation battled in A's mind for a few minutes but the former won out and they carefully investigated. There they found an almost unconscious Person B bleeding that was hiding behind some trash cans. B held a hand to their bleeding stomach (a stab wound was apparent) while they carried a black pistol in the other.
B's vision was impaired as their eyes were blurry and unfocused but they still reacted when they heard A approaching. They threatened to shoot A if they got closer. However, after A identified themselves as the street's 'Angel Doctor', this made B hesitate and in the end they relented. Just as they were about to lose consciousness, A made B promise to not become violent later on. Afterwards, Person A carefully picked B up in a fireman's carry and took them to their clinic where they treated their stab wound.
The next day, B woke up on a patient bed as A entered the room to check on the wound and give it maintenance. Though B had all their weapons temporarily confiscated by A, they accepted the help and as promised they didn't become violent or try any funny business.
Later in the day, Person B's men came to pick their boss up (A allowed them to make a pickup call) and that's when A learned that B was not just a random stab victim, but actually the yakuza leader of the strongest and most vicious group in the city that was currently engaged in a bloody turf war with another group. Oh, well, shit.
A was taken aback and a bit intimidated but B promised that they would not hurt or bother the clinic and instead they offered their support as thanks for A saving B. As such, Person started to B give anonymous donations to the clinic and had their group adopt the area to protect it. Now, while A appreciates the thoughts, they can't help but feel anxious over the whole thing wondering if they can still help patients out without getting dragged into the group's problems. Moreover, it doesn't help that B comes to visit, though in disguise, when A is working at the clinic.
Though initially apprehensive, as A continues to interact with B, they slowly start to bond with one another. However, A was right to be anxious because eventually B's biggest rival hears about what A has done and decides to target them to get back at B's group for their previous loss. As such, A gets dragged into the war and their once relatively normal life at the clinic gets uprooted.
