Boy do I hate this cliche with a burning passion.  Whenever someone is writing a comedy or trying to add some comedy into a more serious story, they always add something that the person thinks is funny.  As a result, they insert this humor into their story, unaware that people have many different tastes in comedy.  They never check with a few people first before inserting the joke, thus causing their audience annoyance instead of laughter.  That is what editing is for, which is something a lot of writers need to do more in order to avoid this cliche.
