Afriqiyah Airways Flight 771, registered as 5 Alpha- Oscar November Golf (5A-ONG), was a scheduled international Afriqiyah Airways passenger flight from Johannesburg International Airport (IATA: JNB, ICAO: FAOR), Johannesburg, South Africa to Tripoli International Airport (IATA: TIP, ICAO: HLLT), Tripoli, Libya, that crashed on the 12th of May 2010 at about 06:01am local time (04:01am UTC) while it was on approach to Tripoli International Airport, about 1,200 meters (3,900 feet) short of the runway, out of the 104 people on board the plane, 103 were killed in the crash, the sole survivor was a 9 year old Dutch boy, the crash of Flight 771 was the third hull loss of an Airbus A330 involving fatalities.
The plane descended below the TW locator 200 feet below the prescribed altitude, the plane descended to 85 meters (280 feet) above the ground when the terrain warning sounded, the Captain ordered a go around, the autopilot was turned off, the First Officer put the nose up for 4 seconds, the plane pitched up to 12,3 degrees, the landing gear and flaps were brought up, shortly afterwards the First Officer made a nose down input which caused the plane to reduce to 3.5 degrees nose down, the go around pitch was not maintained, the flight directions were not followed, the crew were making inputs to the side, this action lead to confusion, the GPWS sounded as the plane lost altitude and the First Officer responded with sharp nose down inputs, the Captain took control and maintained the nose down input, the First Officer was pulling back on his own side stick at the same time, the aircraft was at 180 feet (55 meters), the Captain also pulled back on his side stick, two seconds later the aircraft crashed into the ground at a speed of 485 kilometres per hour about 1,200 meters of Runway 09 outside the airport perimeter, the aircraft was destroyed by the impact and post crash fire.
The summary of the crash was Controlled Flight into Terrain caused by spatial disorientation, pilot error and lack of crew resource management.
