WARNINGS OF A PLANE CRASH AND A MID AIR BREAKUP

Lauda Flight 004, registered as Oscar Echo- Lima Alpha Victor (OE-LAV), was a regularly scheduled international passenger flight from Don Mueang International Airport (IATA: DON, ICAO: VTBD), Bangkok, Thailand to Vienna International Airport (IATA: VIE, ICAO: LOWW), Vienna, Austria, on the 26th of May 1991, the Boeing 767-300ER operating this flight and route crashed, following an uncommanded midair deployment of the thrust reverser on the number one engine (the left side engine), which casued the aircraft to enter an aerodynamic stall and uncontrolled dive and eventual breakup in midair, the crash killed all 213 passengers and 10 crew (223 in total) on board the plane, the crash is the deadliest aviation incident involving the Boeing 767 aircraft, the deadliest of it's kind and the deadliest aviation incident in Thailand's history as of 2023, the crash marked the aircraft type's first fatal incident and the third hull loss of a 767 aircraft.

At 23:17pm local time, the number one thrust reverser deployed while the plane was over mountainous jungle terrain in the border area between Suphan Buri and UthaiThani Provinces because of the reverser design, the left wing became disrupted and resulted in a 25% loss of lift, resulting in an aerodynamic stall, the plane began a diving left turn, manoeuvring overloads the pilots attempts to regain pitch control, in combination with the velocity of the dive it exceeded the structural limits of the plane and destroyed the weakened rear fuselage, taking the rest if the damaged flight surfaces, the loss of the tail caused further loading on the wings as the plane nosed over vertically, reaching a speed of Mach 0.99, breaking the sound barrier, the wings failed and sheared off at the trailing edges, engulfing the remains of the falling aircraft in flames before impacting mountainous wooded terrain and exploding, most of the wreckage was scattered over a remote forest area roughly one kilometre in size, at an elevation of 600 meters (2,000 feet), the wreckage site is about 100 kilometres (62 miles, 54 nautical miles) northwest of Bangkok, close to the Burma- Thailand border, the crash killed everyone on board the plane.

The summary of the crash was an In flight breakup caused by uncommanded thrust reverser deployment; due to possible design flaw and electrical failure.
