Ultimate Cell is a form which is exclusively created by Dr. Gero for the Cell from Future Trunks' timeline.

Future Cell was one of the many people who was revived via the Super Dragon Balls by Towa, Mira, and Fu during their deal to help Android 21.

Future Imperfect Cell learn that a counterpart of his had killed Goku but was killed by an 11 year old Gohan which cause him to think for Perfect Form is deeply flawed.

Gero secretly made this form for Future Imperfect Cell which he can gained by absorbing Android 21 similar to his counterpart did to Android 17 and 18, but with Cinos protecting his love interest 21 which mean Future Cell have this form for long.

His Ultimate form being not intimidating and small is very similar to Frieza's true form/ 4th form.

Obviously the body is based on the Hunter X Hunter character Meruem due to the character being similar to Cell in appearance and I heard they say the character is a rip-off of the famous DBZ villain.
