Akko had messed up. Again. No surprise.
What happened was that she was attempting yet another spell, one with a messy aftermath. It only worsened when Akko's girlfriend, Diana, entered the perimeter and was hit with the effect of this spell.
Both girls were knocked out, and when they woke up, Akko found herself inside the body of Diana.
The same was with Diana, only she was in Akko's body.
"Diana?" "Akko, what did you do?" "I'm sorry! Please forgive me!" Diana pinched the bridge of her nose.
"I'll go find some way to fix this. Wait right here."
While Diana left the room, Akko stood up and caught sight of her reflection in the mirror.
She leaned in closer, admiring her facial features.
Seeing as she was inside Diana's body, this was good, because Akko now got to see every little detail of her girlfriend's face. Every strand of hair, the hues of the eye color, and the smooth, fragile skin.

She turned the pages, and found what she was looking for.
Diana picked up the book and ran back to her girlfriend's room. "Akko, I've found-!"
