He had drifted into the wrong lane and was about to collide head on with the car until Cas grabbed the steering wheel, turning it swiftly to swerve out of the way.
Everything happened so quickly, Dean barely had a chance to register it all. He heard tires screeching, glass shattering, and screaming in the distance.
When everything settled, he took a look around. It wasn't a head on collision, but it was still pretty bad. The two cars managed to slam into each other's side. The Impala's right side and the other car's left side. Cas's window was shattered, and Cas was unconscious in his seat, the shards of glass piercing the skin in his face and neck.
"Cas?" Dean's voice broke. "Cas!"
They were taken to the hospital. Other than a few cuts and bruises, Dean was fine. But Cas hit his head pretty hard, and all the glass caused a lot of blood loss. One of the shards was a centimeter away from his jugular. It was a serious close call.
Cas was in there for a while, Dean constantly by his side. Sam was there too, of course. They were both a bit worried for him.
"He'll be fine, Dean. He'll be outta here in no time." Sam would reassure.
"Sam...-" He let the sentence hang.
"What is it?"
"What he was saying... in his sleep... in the car... he kept saying he was pretty much dead inside."
"Dean, it was the medicine. I'm sure of it."
"You don't get it, Sammy. He... he just went on and on about... how he died when he lost his grace. How it was... how it was my fault that he fell. That he fell for me. He fell for me."
He let out a shaky breath, fighting his tears. He let one slip, which was a small crack in his dam of false bravado.
"Just don't overthink it. Try and get some rest." Sam tried to placate his brother to no avail.
"Does he love me? Or is it just me? Am I just seeing what I want to see? Or... what?"
"I don't know, Dean. I don't know."
The moment Cas awoke was the moment Dean's spirits were lifted.
"Cas, you okay?" He asked as he jumped out of his chair, standing over his angel.
"I'm fine. My head just hurts, that's all."
"Cas... what you were saying in the Impala... what-"
"Dean... I'm sorry. I was... out of it. My mind couldn't function properly. I really am sorry, Dean."
"Don't be. It's okay. Is it true, though?"
"Is what true?"
"Do you... really think... I'm the reason you... you fell? Did you really fall... for me?"
"Yes," He answered without hesitation. "Dean, I fell for you in ways you cannot begin to imagine. I fell for you in every sense of the word. Dean... I am honored to say that you are the reason I fell."
Dean smiled as a tear of joy trickled down his freckled cheek. His angel loved him back. That's all he ever wanted.
"I love you so much, Cas."
Cas smiled back. "I love you too, Dean."
Pretty soon, Cas was well enough to go. Dean decided to let Sam drive, since he didn't trust himself behind the wheel at the moment.
He sat in the back with his angel and they held each other close, Cas's face nuzzled into the crook of Dean's neck. They sat back there with their fingers intertwined until they both fell asleep.
A while later, Dean awoke and found that they had arrived at the bunker. He looked down at his sleeping little angel and smiled. He loved the little baby in a trench coat. His baby in a trench coat.
He carried him in his arms bridal style and followed Sam inside. He took his angel up to his, now their, room.
