I bet Dean is still mad about the whole April incident. I bet he blames himself for that too, and thinks that if he hadn't turned Cas away to help Sam, then Cas would never have gone to April in the first place, and would never have been killed by her.
And I bet Dean is still forever pissed at April, because he betrayed Cas's trust when he made him leave the bunker, and Cas turned that trust to April, who took advantage of it.
