Castiel was waiting patiently in the hospital lobby. Noises of mother’s crying and hospital nurses screaming didn’t seem to penetrate his thoughts. All he could think about was that Dean had called him. Dean had been his first call. As his friend was broken and laid out across the high way, Dean’s first call had been to Castiel. Fresh tears stung his eyes as he hunched over in his seat and felt like the world was some comically sick joke. Twenty years ago, when the two sat outside Dean’s house and cut their palms to become blood brothers, Castiel never thought that their bond would end. He never thought that one day Dean could slip through his fingers. But tonight, reality came crashing in on him as he waited to hear if Dean would make it.
Six Hours Ago
Castiel was patiently shelving some Chex Mix at the almost void grocery store where he worked. It was a mindnumbingly boring day that he hoped would end with a hot bubble bath and maybe some reruns of Desperate Housewives. Through the silence of the store, he heard his ringer going off from his pocket. Grabbing it from his pocket, he saw it was Dean (the name on his phone read ‘Batman’). Cas smirked and hoped Dean would maybe get some laughs out of him and help him make it through till quitting time.
“Dean?” Cas spoke into the phone as he brought his shoulder up to cradle the phone so he could keep shelving. “God I hope-“
“Cas.” Dean’s voice was wrong. It was weak and it sounded like he was choking on something.
Immediately Cas dropped what he had in his hands and stood motionless as dread poured through his blood. Something wasn’t right. “Dean? Dean, what’s wrong?”
“Cas.” A low breath that hitched. Then a slow moan. “Cas, I need you. I-I’m cold.”
“Where are you?” Cas nearly tripped over his inventory as he quickly bolted to the front of the store. Dean was never like this. “What’s wrong?”
“Some asshole-“ Cough. “Some asshole hit my car, Cas. I swear to God-“ Low moan. “She is totaled.”
“Where the hell are you Dean?” Castiel waved to his manager and pointed at his phone with a look that certainly conveyed the weight of the situation. His manager nodded and waved him along, it wasn’t a busy day. Cas didn’t hesitate as he shoved open the doors and started to frantically run toward his car. “Are you okay, Dean? Talk to me.”
“I need you, Cas.” Dean’s voice was low now, fading. “I can’t feel a goddam thing. I’m so cold. I-I need you here. I don’t want to go without seeing you, Cas.”
“Dean, don’t talk like that, alright? Stay with me. Listen to my voice. Where are you?” Castiel fumbled with his keys as he opened his car and in seconds he was driving down the road to nowhere. He had to find Dean.
“I can see the diner from here.” A pause and another choked moan. “People are starting to come… I think they are calling the cops. Cas, please, Cas, listen to me. I don’t think I have much time. I can-I can feel the coldness.”
Tears fogged Cas’ vision as he whipped the car in a harsh U-turn and started to head toward the diner that had been their hang out after school so many years ago. Dean couldn’t die, Cas wouldn’t let him.
“Cas, I need you.” Dean’s voice wavered and Cas could clearly hear the soft sobs. “I love you, Cas.”
“Dean, Dean don’t you dare fucking die. I am on my way, you hear me? I am going to be there in minutes. I promise you that you aren’t going to die. Listen to my voice, focus on me. I love you, too, Dean. And I am not going to let you die.”
“I need you.”
Present Time
Castiel felt his entire body sink. It was like he was waiting for judgment day. Dean was in there, on a slab, with doctors casually talking about if he would survive. If Dean was gone, Cas didn’t think he would find meaning in life any more. Every day he looked forward to Dean’s eyes, and Dean’s jokes, and Dean’s smile, and the way Dean made everything seem so much brighter. Without Dean, Castiel would be in hell.
“Are you Castiel Novak?” A pleasant voice breached Cas’ darkness.
Cas stood and wrapped his arms around his chest in an effort to comfort himself from the impending bad news. His eyes were met by a lovely nurse who smiled at him with warmth that Cas needed in that moment.
“I am.”
“Mr. Winchester has been asking for you. His condition is stable, a few broken bones and a concussion. The internal bleeding has stopped. We will be keeping him here a few days for observation, but he will be okay.”
Castiel felt the breath being sucked from him as he sighed and relaxed himself. His Dean was going was going to live. “Thank you.”
“Don’t thank me. I am just a nurse.” The nurse peered down at a clipboard in her hands. “Is there any family we can call?”
Cas shook his head. “His parents are dead and his brother is…” Cas paused with a slight pang in his heart. “His brother is in school, but they hadn’t left on such good terms. I am all he has.” The weight of those words hit him hard.
The nurse nodded. “Follow me, then.”

Dean’s eyes were closed and his chest rose and fell in the smallest way that made Cas’ world crumble. He had never seen Dean hurt like this before, he looked so broken and so vulnerable. It made Cas’ stomach lurch and his back ache. But he stepped forward into the room nonetheless because he needed to be close to Dean. He needed to know he was really there.
“Dean?” Cas whispered as he stood against the bed until his thighs were pressed to the fabric.
A small smile cracked along Dean’s lips. “Heya, Cas. How’s it going?”
“I nearly had a heart attack, Dean. I thought-“ Cas reached his hand out and enveloped it over Dean’s. “I thought I was going to lose you.” Tears streamed down his face and padded against the bed sheet as he squeezed his best friend’s hand. “You really scared me.”
“You know I am not going anywhere. I am invincible.” Dean let his head fall to the side but still didn’t open his eyes. Small bruises covered his cheeks and Cas yearned to lean over and kiss them away. “I sure as hell ain’t leaving you.”
Cas breathed out his nose as the tears wouldn’t stop. “Why didn’t you call an ambulance, Dean? You were so careless.”
“I needed to hear your voice.” Dean sighed deeply and frowned lightly. “I needed you.”
Cas collapsed against the bed and pressed his forehead against Dean’s arm. Their fingers intertwined and Dean latched on until Cas thought his fingers were going to break. “I love you, Dean.”
“Yeah, love you, too.” Dean shifted and soon Cas felt an arm wrap around his shoulders to steady them. “Truth is, I loved you since forever. And if I was going to die I had to let you know that.”
Castiel lifted himself onto the bed and curled against Dean’s side with his head pressed firmly under Dean’s chin. Breathing was restricted, but he could care less. “I will always be here, Dean.”
“You’re my angel, Cas.” Dean kissed the top of Cas’ head and held him in that hospital room for hours. Neither one of them letting up even as the doctors came and went. And when Dean was checked out, Cas took him home and held him. Whenever Dean needed Cas, he was there like a guardian angel.
