Doctors practice deception all the time.

We give vague answers to hard questions. We don't talk about post-op pain.

We say, you'll experience some discomfort...

When Derek Shepherd's clinical trial for Alzheimer's officially took off, Alex Karev was surprised that he was the resident selected to be at his side. He didn't frequently work with his brother-in-law, but when he did, Derek was constantly impressed by the amount of passion that Alex poured into his patients and work.

Alex knew that the trial would take more time out of his schedule, but it would also look impressive on his record. He only hoped that Derek, of everyone, would make sure Alex still had enough time for his son at home.

The surgery went well.

But the placebo has to be the doctor's greatest deception.

Outside of the OR, Alex attached a piece of paper to the door, stating that the patient would be awake during the procedure. And then, when he went back into the OR, he went ahead with handing Derek the envelope, which said whether or not the patient would receive the placebo. A decision that was chosen at complete random.

Even though it was early in the morning, Mallory Shepherd and Meredith Grey sat up in the gallery, watching as their husbands operated. Mallory had Carter in her lap as she rubbed her eyes, sipping on her coffee just to stay awake.

Half of our patients, we tell the truth.

"Are you tired?" Meredith questioned as she watched Mallory yawn once again. "I don't think in the years that I've known you that I've ever seen you tired. You're usually wired with energy once you step foot into the hospital."

"I wish I was," Mallory groaned in response. "I've been exhausted lately. You'd think I was the one on the clinical trial."

"Well, you are the one who's up with Carter," Meredith pointed out. "And you're even here to watch Alex operate."

"Yeah, I guess you make a good point," Mallory agreed as she shrugged her shoulders. "I don't know."

Carter, who was sitting up straight on Mallory's lap, suddenly pointed down into the OR as he exclaimed. "Dada!"
