It was early in the morning when Mallory Shepherd and Alex Karev drove back from Alex's hometown in Iowa. They drove through the night, speeding most of the way there in hopes of making it to their shift on time, making use of their two days off without anyone having to question it.
They had left Seattle in a rush two days prior to deal with Alex's younger brother, Aaron, who had been diagnosed with schizophrenia after an incident where he had tried to go after Amber. Mallory and Alex left as soon as they got the call, making the decision to leave Carter back at home.

At the same time, Louis Carmichael had gone on a spontaneous trip to New York, insisting that he had to check in on some maintenance in one of his buildings. Since he was gone, they decided to resort to April Kepner, who had been begging to babysit. She was also the only one not to question their sudden trip.
When they returned, Mallory had a visible bruise under her eye from when Aaron had an outburst, and she had gotten in the way. For the entire drive back to Seattle, Alex's eyes stayed on the bruise while his hands gripped the steering wheel tightly.
"Are you sure that you're okay?" Alex was the one to break the silence. "I mean, that was a lot. Especially for it being our first trip there—"
"It's fine, Alex," Mallory answered with a nod. "Really."
"It's not fine," Alex responded by shaking his head. "I don't like it when you say that it's fine."
"Well, it is," Mallory made sure to insist. "It's not like Aaron was in the right state of mind, I mean, if he was, he never would've done any of that. We're his family; we support him no matter what happens."
Alex kept his eyes on the road as he started to mutter. "I should've had you stay at home—"
"Enough with that, okay?" Mallory was quick to object. "I'm your wife. Your brother needed help, your sister needed support. There is no world where I would've stayed home."
It's important to keep this pressure regulated.
"Have you heard from Kepner?" Alex changed the subject to question. "I don't know if I like that she's had Carter."
